Double Chin: Liposuction or Neck Lift? How to Tell Which You Need
Whether a double chin needs liposuction or a neck lift depends on what is causing the fullness. If the problem is fat beneath the skin and your skin is still elastic, liposuction (fat removal) or laser-assisted lipolysis is often enough. If the skin has loosened, if there are vertical muscle bands in the neck or if the jawline has started to sag, removing fat alone will not correct them, and a neck lift is discussed instead.1,2
Choosing the wrong one has a cost. Removing fat from loose skin can leave the neck thinner but looser.2 On the other hand, a neck lift on a younger neck that only carries extra fat is a larger operation than necessary. This guide explains a simple mirror check, how each finding changes the decision and where non-surgical options sit.
In short
- Elastic skin, superficial fat: the best situation for liposuction or laser lipolysis.2
- Loose skin, muscle bands: a neck lift is needed; non-surgical treatments cannot achieve the same result.1
- Skin quality decides: thin skin that has lost elasticity may look loose after fat removal.2
- Injections are a non-surgical option: in a large trial of a fat-dissolving injection, 66.5% of treated patients improved by at least one grade, compared with 22.2% on placebo.3
- Keep your weight stable: liposuction is not a weight-loss method, and later weight gain can change fat distribution.2
A simple mirror check
This does not replace an examination, but it gives a first idea of whether the problem is fat or skin. Look at yourself side-on in a mirror and try the following:
- Pinch: hold the tissue under your chin between thumb and forefinger. If you are holding a thick, soft layer, fat may be playing a major part.
- Tense: clench your teeth and pull the corners of your mouth down. If vertical bands stand out at the front of the neck, the muscle (platysma) may also be involved.
- Skin: gently pull the neck skin and let go. If it returns slowly or forms folds, its elasticity may be reduced.
- Profile: if your chin is set back or small, the profile may change less than you expect even after fat removal.
At examination, whether the fat lies above or below the neck muscle, the jawline and the position of the voice box are also assessed.
When is liposuction enough?
Liposuction removes fat beneath the skin, above the neck muscle. Mayo Clinic shows the area under the chin as one that can be treated with liposuction, and notes that firm, elastic skin gives smoother results while thin, inelastic skin may look loose afterwards.2 In laser-assisted liposuction, a laser fibre passed through a small cut breaks down the fat before it is removed.2 We describe the technique on our double chin surgery and laser lipolysis page.
Liposuction is not a weight-loss method, and if you gain weight afterwards, fat distribution can change.2 Serious complications after liposuction under the chin are rare. A review collecting published cases reported a hollow under the chin, swelling (oedema), thick or tight scars, temporary weakness of a facial nerve branch and isolated cases of serious infection; however, only four studies met its criteria, so there are no reliable figures for how often these occur.5
When does a double chin need a neck lift?
The American Society of Plastic Surgeons describes a neck lift as treating excess fat under the chin, loose neck skin and muscle bands in the neck.1 In other words, it addresses not only the skin but the muscle and, where needed, the fat beneath the muscle. Fat below the muscle cannot be reached with a liposuction cannula; it needs surgical access through an incision under the chin.
If jowls have also formed along the jawline, the problem is not confined to the neck; ASPS also lists fat and skin laxity in the lower face that creates jowls among the concerns a neck lift addresses.1 Where jowls clearly break the jawline, a facelift is often planned together with the neck lift. We explain when a neck lift is done alone and when it is planned with a facelift in neck lift alone or with a facelift.
| Liposuction / laser lipolysis | Neck lift | |
|---|---|---|
| What it treats | Fat above the neck muscle | Fat, loose skin and muscle bands1 |
| What it does not treat | Loose skin, muscle bands, fat beneath the muscle | A small chin due to bone structure; fine lines on the skin surface |
| Incisions | A few millimetres under the chin | Under the chin and often behind the ears |
| Anaesthesia and stay | Usually local anaesthesia, same day | General anaesthesia; usually one night in hospital |
| Time in Istanbul | A few days | About seven days |
| Recovery | Swelling eases within a few weeks; final contour over several months2 | A timeline similar to a facelift |

Fat-dissolving injections and other less invasive options
There are also smaller interventions than conventional liposuction. A review lists laser- or radiofrequency-assisted lipolysis, deoxycholic acid injections and device-based treatments among current minimally invasive or non-surgical options for fat under the chin.4 In a large phase III trial of deoxycholic acid (a substance that breaks down fat cells), 516 patients were randomised to the drug or a placebo (an inactive injection). Twelve weeks after the last treatment, improvement of at least one grade on both clinician and patient scales was seen in 66.5% with the drug and 22.2% with placebo; improvement of at least two grades was seen in 18.6% vs 3.0%, and most side effects were confined to the injection site.3
Those results were obtained in people with moderate or severe fat under the chin, and follow-up was limited to 44 weeks.3 Injections do not tighten loose skin or muscle bands. ASPS likewise notes that non-surgical treatments cannot achieve the results of a neck lift, although they may delay the point at which one becomes appropriate.1
Double chin liposuction or neck lift: other factors in the decision
- Age alone does not decide: one person in their forties may have loose skin, while another in their fifties still has good elasticity. The decision follows skin quality, not the calendar.
- Weight plans: liposuction is not a weight-loss method;2 if you plan to lose a significant amount of weight, it usually makes sense to wait.
- Chin and bone structure: a receding chin can leave a short-looking neck profile even after fat removal, and is assessed separately.
- Expectations: liposuction is the smaller procedure but gives a more limited change; a neck lift is more extensive and takes longer to recover from.
Time in Istanbul
Fat removal alone is usually a same-day procedure, and a few days in Istanbul is enough; the exact number is confirmed after consultation. When it is combined with a neck lift or facelift, one night in hospital is usual and about seven days are planned. In Op. Dr. Esin Özlem Atmış’s planning, reviews take place at 1, 3, 6 and 12 months; if you live abroad, they can be held online where appropriate, and an in-person examination is requested if needed. Seek urgent medical help without waiting if you have heavy bleeding, rapidly increasing swelling or difficulty breathing after the procedure.
Frequently asked questions
Which is better for a double chin, liposuction or a neck lift?
Will my neck sag after liposuction?
If the skin is elastic, it usually adapts. Thin skin that has lost elasticity can look loose after fat removal.2
At what age do I need a neck lift instead of liposuction?
There is no fixed age. The decision depends on skin elasticity, muscle bands and the jawline.
Can injections replace liposuction for a double chin?
They are a non-surgical option for excess fat; in one trial 66.5% of treated patients improved by at least one grade, compared with 22.2% on placebo.3 They do not correct loose skin or bands.
Will liposuction get rid of the vertical lines in my neck?
No. Those bands come from the muscle and are corrected with a neck lift.1
Does the fat come back after chin liposuction?
If your weight stays stable, the result is usually maintained. If you gain weight, fat distribution can change.2
What are the risks of chin liposuction?
When will I see the result of chin liposuction?
Swelling usually eases within a few weeks, and the area looks slimmer over several months.2
Questions to ask at your consultation
- Is the fullness under my chin caused by fat, skin or muscle?
- How much of the fat sits above the muscle, and how much below it?
- Is my skin elastic enough for fat removal alone?
- Will my chin or bone structure affect the result?
- Should I wait if I am planning to lose weight?
- How many days should I plan in Istanbul?
